How to Enroll Your Child

2. Complete Enrollment Packet
Fill out the required student registration forms, immunization records, emergency contact cards, and tuition agreement paperwork.
3. Welcome to Tender Hearts!
Confirm your start date and bring your little one for their exciting first day of friendships, growth, and learning adventures!
What to Prepare for Enrollment
To ensure state compliance and the health and safety of every child at Tender Hearts Dayschool, we ask parents to provide the following items prior to the first day:
- Completed Enrollment & Emergency Contact Form
- Current State Immunization Certificate (CIS form)
- Signed Tuition & Dayschool Policy Agreement
- Allergy & Dietary Preference Form (if applicable)
